In order to participate in athletics at East Ridge High School the following steps must be completed prior to the first day of tryouts.
- A physical exam must be completed by a licensed physician, a licensed osteopathic physician, a licensed chiropractic physician, a licensed physician assistant, or a certified advanced registered nurse practitioner
- The FHSAA EL2 physical exam form must be completed by this practitioner
**No other forms will be accepted
**The EL2 form is only valid for 1 calendar year from the date of practitioner’s signature
**Physical must be valid through the entire season
- An account must be created on Athleticclearance.fhsaahome.org
**Please click on all sports that you intend on trying out for
**Please place the correct name for the signatures. Parents name must go where it says parent signature and students name must go where it says student signature.
- The following forms must be uploaded to your athletic clearance account
- EL2 physical form
- Birth certificate
- Insurance card
After this information is complete it will be reviewed by the Athletic Trainer. This is done closer to the start date of each season. If done correctly you will receive a confirmation email stating that you are cleared for that sport. If this is not done correctly you will received a denial email stating what needs to be fixed.
